Frequently Asked Questions (FAQ) - New Case Submission Eligibility
 
Q: What if an employer fails to submit required documentation and is deemed ineligible according to the plan's eligibility requirements?

A: Most carriers require certain documentation for proof of business. The documentation generally is very similar between carriers and is located on the reverse side of the Broker Group Census Forms. In addition to documentation required by the carriers, SBSB also requires documentation for membership submission. If an employer cannot provide the necessary documentation for proof of business, you should contact SB TeleBrokerage for assistance. If the documentation submitted to SBSB clearly does not meet the requirements of the plan, SBSB may not be able to submit the case. In some questionable cases SBSB will confer with the plan directly for approval or denial of the case.
Q: I have a new business case that has four active employees and one COBRA person. Is the COBRA person eligible to join with the other employees?

A: Yes, as long as the employee with the COBRA or Mini-COBRA has had continuous coverage since their qualifying event. Employees who elect COBRA or Mini-COBRA have the same rights as active employees - including changing carriers or products under a carrier on their anniversary.
Q: Who do we contact if an employer group changes their business address or phone number?

A: Please notify our Member Service Department of any changes by calling 1-800-472-7199 or by writing to Small Business Service Bureau, Inc., National Operations Center, 554 Main Street, P.O. Box 15014, Worcester, MA 01615. We will notify the carrier of all changes.
Q: What if I submit a new business case and the necessary documents are not received by SBSB until after the requested effective date? Can I still have the date requested by my client or does the date have to be pushed up to the first day of the next month?

A: If SBSB does not receive the necessary documents to submit a new business case to the plan prior to the requested effective date, the date may be adjusted to reflect the first day of the next month. Please note necessary documents may include items such as waiver forms for employees waiving coverage, completed company census form or other documents deemed required by the plan to establish a new business' eligibility. Few carriers allow assignment of effective dates retroactively.
