| Small Business TeleBrokerage will pay a bonus of $16.00 per subscriber, per month for any newly acquired subscribers enrolled from new group sales of Fallon Community Health Plan (FCHP) in the 1-4 employee market. The $16.00 bonus will be paid in addition to the $10.00 service fee currently being paid by Small Business TeleBrokerage. The new bonus program is effective for new FCHP group sales with effective dates of July 1, 2007 through December 1, 2007. |
