As you know, due to Massachusetts Health Reform, most Mass. adult residents (age 18 or older) are required to purchase health insurance by 7-1-2007. To avoid paying a penalty, residents will need to provide evidence that they have health insurance by December 31, 2007 on their 2007 MA Resident state tax forms.
 
The 1099-HC Form is a new Massachusetts documentation requirement that serves as proof of health insurance coverage for Massachusetts (MA) adult residents and must be used to complete the Schedule HC form, a new required tax return attachment for all MA residents. Harvard Pilgrim Health Care will complete and mail, at no cost, the 1099-HC to Massachusetts’ resident subscribers by January 31, 2008. All Massachusetts’ resident subscribers will receive the 1099-HC, this includes subscribers from both fully and self-insured accounts.
