Fallon Community Health Plan Extends Bonus Program |

- Small Business TeleBrokerage will pay a bonus of $16.00 per subscriber, per month for any newly acquired subscribers enrolled from new group sales of Fallon Community Health Plan (FCHP) in the 1-4 employee market.
- The $16.00 bonus will be paid in addition to the $10.00 service fee currently being paid by Small Business TeleBrokerage.
- The extended bonus program is effective for new FCHP group sales with effective dates of January 1, 2008 through December 31, 2008.
