| Tufts Health Plan will honor renewals through October 31, 2006. Effective for November 1, 2006 renewals, Tufts Health Plan will no longer offer a plan design under the name Liberty by Tufts Health Plan, powered by Destiny Health. |
| If a client decides to renew their plan for another year, please follow the normal Liberty by Tufts Health Plan, powered by Destiny Health renewal process. |
| If a client decides to switch to another Tufts Health plan, they will renew again on April 1, 2007 with all other groups. Please issue the standard Tufts Health Plan Employer Group Agreement that reflects the 4-1-07 open enrollment date instead of the Liberty by Tufts Health Plan group agreement. |
